I got a form check on my deadlift a week ago from an SSC, Andrew Lewis, which has been really helpful. So, I decided to get a form check on my squat, too.

Sorry, I know it's not the best angle or height, but it was the best I could get.
I was cueing myself knees out, hip drive, and stay in the hips.
Feet need to be just a little wider. Bend over more and cut the depth off a half inch.
Your elbows are way cranked up. Think about getting your chest up and elbows under the bar at the top. See if a narrower grip is possible/helpful.
