335x5 - what am I doing wrong? (the bar comes off my leg at the beginning of rep 5, i think its a balance issue?; a little concerned i'm not maintaining ideal back extension)

You aren't setting your back before you push the floor, you're doing some kinda combo. Set back, pause, push the floor. Embedded in that is you dropping your ass. When you set your back, your butt should not go down. You shouldn't need straps but that's of lesser importance.
