Lean forward more
Send knees out early and freeze
Cut depth off a bit higher

Coach, wasn't the bar progressively rolling upwards?
It's rolling up just a little, but settling back each rep.
Haider, you'll need to keep you pecs up just a little more to get your back tight to fix that.

A good Rule Of Thumb is that if the bar rolls up, it was too high on the back.
I will try being more mindful of the bar placement the next time. Hopefully that will also help with the knee slide as pointed out by Jovan. Will post a follow up with in 24 hours.
You'll have to excuse the shoes just this once coach Andrew. I ordered a pair of Adipowers but it will be a while before I recieve them.

Kept the bar a bit lower this time and that seems to have fixed the bar riding up issue. The knees are still a bit of a challenge but will fix them with a little bit more work.
