Why does the bar move slightly forward as it leaves the floor?

Took some advice from last time I posted, increased by 10lb. Felt better. Working on getting my chest up, my back flat and not dropping my ass. I can see form degrade on the last rep, just gassed mostly. Any tips on the first 4 or how to get through 5 after a full workout, greatly appreciated.

I don't know. My only guess would be that I'm unconsciously keeping the bar off my shins to avoid, what I assume would be, pain.
Should I just wear high socks or get comfortable with scraped shins?
You should extend your knees as the initial push off the floor. This is what the cue to "Push The Floor" is for.
