    You're not finishing your chest-up shoulders-back lockout at the top.

    As part of the "squeeze your chest up/set your back", you are dropping your hips. It looks like you're combining steps 3 and 4. Do these separately. Put your hips into position (by moving your shins to the bar), then, without moving your hips are all, squeeze your chest up/drop your belly between your thighs. Your back comes out of extension on a few of these because I think you're less volitionally extending and more "rocking back" into the start of the rep. Squeeze your back into extension and HOLD IT.
