    Stance is way too narrow, depth is excessive, and you are looking up every rep. The first two are related.

    The bar needs to be like a half inch lower on your back. Your back angle is changing all the way down because the bar isn't quite in the right place: it kind of "rolls forward" and your hands follow it.

    A general rule for when under stress is, don't try not to do something. Find something else to do instead. Instead of trying not to raise your head when the rep gets hard, try cueing yourself to keep your chin tucked, or to LOOK DOWN to correct it. The latter has always worked for me.
