    The bar slopes down and to the left, it's the most obvious at the bottom of the squat and I can't figure out why. It's causing pain in my left hip and left shoulder. It's been getting worse. I think I might have a leg length discrepancy, I have an appointment with a podiatrist to get measured. If you see anything wrong with my form please let me know.

    Your grip may be the problem. With your hands flopped on top of the bar and your wrists in flexion, your grip cannot support the position of the bar during the set. And only the last rep of the set allows us to see the bar position and the hip angles.

    In addition to your grip being wrong as Rip said (which is contributing), the bar is like an inch off center.
