2 weeks on novice linear progression.
Failed last rep in sets 1&2. Had to re-rack and repeat last rep.
This is the 3rd set with a narrower stance. Form doesn’t feel right, looking for tips.

Ted, the Smith machine is not part of the program, because it is in no way your friend. Don't use that trashy thing. You need an unrestrained barbell and a a regular rack for doing the exercises in the method.

There is a universe where the 2D smith machine could execute something like a freestanding squat. But then, if it does, the Smith machine is really only an impediment, isn't it? It doesn't even have the advantage of the hooks here.

The Smith machine prevents side to side motion of the barbell, which, while probably not a primary training stimulus, is a rather large part of the movement, which are related to your deficiencies here. The work done to prevent lateral motion is the work which keeps you "tight" under the bar, which is what you are failing to do.

None of these are to correct depth, the bar is in the wrong spot on your back, and your back is not rigid. Read the book and pretend you are using a real barbell.

Thank you all for your replies. Just started reading the book. This is an apartment complex gym and probably they got this 2D smith machine to reduce the liability of someone dropping a barbell onto the floor. I incorrectly assumed that it would be equivalent to an unrestricted barbell.
