These are not bad at all, with the exception of the lifting of the chest early at the top of the movement, as you noted.
I opened another thread (Knee sleeves causing patellar tendonitis?) and wrongly attributed my previous knee tendonitis to knee sleeves as I haven't had any knee pain without them but Rip soon put me straight and realised it was obviously a form error that could've been caused by wearing knee sleeves.
Video of last set of 5 squats in the link below at 140kg. During setup I positioned the camera at my hip....whilst standing, doh! I've taken other videos of myself not worthy of being posted and I was below parallel and knees are in line with my feet. The only obvious issue I see with these is not driving my hips throughout the whole range of motion.

I'm pretty sure that the rack you're using will allow you to put the hooks on the other side, so you're facing out and away from the mirror as you squat. Doing this might help you with keep your chest pointed down longer, since it removes the insidious lure to look up.
It does, and I've actually had this thought but unfortunately with the amount of people in the gym I think it would be more distracting than the mirror. I've definitely found myself looking at the mirror on harder reps as I come up!
I know what you mean - I've found that it helps to have a small, specific thing to focus on. From what shows in the mirror, I think you have the space to set something brightly colored down on the floor in front of the rack, and use that.
Of course, I see what looks like chalk residue in that rack, so a chalk mark on the floor might be just the thing. (Children's sidewalk chalk, if you want a bright color...)
Just make sure you clean it up when you're done as a matter of general gym etiquette, and there should be no cause for complaint against you doing so.
