Watching high-level powerlifters (in competition, doing a single rep) it seems like the majority of them breath and do the valsalva maneuver while standing up before bending over to grab the bar. I've never tried this and always setup and breath at the bottom for the first rep in a set as well as between reps. I tend to fidget around a bit getting a good grip, pulling the slack out of the bar, and setting my back before actually pulling, so if I wanted to breath before the setup I would probably have to get faster at all of that if I want to stay conscious.
How do you guys do it?

As far as seeing what elite or high level lifters do, you should take it with a grain of salt. Most of these lifters are genetic freaks and of course have been doing this for a pretty long time. What often works for them won't really work for us. As for the breathing, just do what works for you.
I breathe, shins forward, chest up, and pull.
I normally take a big breath, squeeze my chest up and tighten my lats all in one motion. Then I rip it off the floor.
I've tried the method of breathing at the top, but my sets always take wayyyyy to long that way.

Set my feet, hands on the bar... everything else relaxed. Then a deep breath and:
Conventional: shins to the bar, lats engaged, pull
Sumo: ass lowered, lats engaged, pull
