Default squat fail - weight or form?

After 2 years on posting on this site, I finally got a "good" video of my squats.

Rip, here are my 3 squat attempts from my recent meet. I would like to ask you for your opinion especially on my 3rd attempt. Is it technical or just too damn heavy? (took a 20kg jump by the way because I want to shoot for the stars and my second attempt was only 10kg more than my raw max)

I think I may have gone extra deep and I dive bombed the descent. Your thoughts?

I don't see anything in the 3rd attempt that was inconsistent with the first two. Your descent is a little fast, but I think this was more a case of an incorrectly-picked 3rd attempt weight.

If I may chime in, there was also no attempt to use a valsalva. Letting go of all that air right at the bottom of the squat will kill any chance of making that kind of attempt.
