    You're rocking back and pulling off your heels, and dropping your hips during the set-up. THis is evidenced by: being able to SEE you rokcing back on your heels, and I can hear the weights rattle as you start the pull, which means you didn't pull the slack out of the bar - you took the easy way out and dropped your hips. Don't.
