Looks like your front squating the weight up to me
My knees are coming in a bit out of the hole, even as I try to push them out. I know that squats near 5RM are going to be a bit messy, but is the amount my knees move still alright for a heavy set? Would a wider stance help keep them out?

Weird angle + all black probably.
Your feet are externally rotated too far out. Point your toes a little more forward and that will likely fix the knee wobble you have going on. Its hard to push the knees out and keep them there with your feet angled out too much, youre in a weak position for the glute med to do its job.
To me it's acceptable, as long as you try to minimise it, and it doesn't get worse.
Also, a side vid would be nice, as it looks like you're a few inches high.
