    Couple of thoughts:

    1) Weight is too heavy, I recommend dropping to about 105lbs, and make small incremental jumps, just like we teach every one else.
    2) Depth is simply not there. Review some of the other form checks. The crease of your hips must drop below the top of your kneecap...or it's not a squat.
    3) Your video is titled 5 rep max...I do not recommend attempting a 5RM at your age. Work the program, 3 sets of 5, adding small amounts of weight each workout, just like the book says.
    4) I admire your tenacity and desire to get stronger at such a young age.

So, drop the weight, squat with perfect form, and make small gains. In a few years the hormone profile in your body is going to change, and the foundations you are laying down today will help you grow, grow, grow.

    Keep up the good work.
