I'll try. You are doing a high-bar squat--did you meant to? Read the part in the book on bar placement, and also check your hands. With your wrists extended like that, you'll eventually get elbow problems.

Looks like your knees are sliding past your knees too much. The could be due to the high-bar squat. Focus on keeping your legs tight, and really sticking your butt back. Otherwise you will be "leg pressing" your squats like I was.
