Is the cue for the high bar squat, "lead up with chest" (like the front squat), or is it "drive the hips" like the low bar squat? Or is it a balance of both? Thanks.

Squat down, bounce and then just push...

It should still be hip drive. The difference will be your center of gravity and whatever cues you need to use to keep your back up with the bar a little higher. I don't think there's any "lead with the chest" with any squat where the weight is behind your shoulders.

Knees out, chest up, drive through heels.

The cues that I find people need most often with high bar are "chest up" and "hips back under," which are what I cue a lot for front squats.

Look straight ahead, attempt to keep you back vertical (it wont be vertical, just try to do it). Everything else is the same.

Chest up, heels. Coming up out of the hole is more or less literally just standing up. I like to say "stand up as if you're going to jump," i.e. stand up powerfully.
