I was guilty of this because I tore my adductor squatting when I was still learning the lifts and never repositioned my toe and knee position after it healed. When I eventually started externally rotating the femurs again all of my issues off the floor vanished quite quickly. The wider grip took some getting used to but I forgot about it rather quickly.

In the second month of my NLP training, I suddenly felt soreness in my low back during power clean and DL.
I read the blue book for troubleshooting, and noticed that I never consciously keep my knee out.
When I actively set my knee out, I felt my belly drop (almost like slip out), and problem solved.

A friend of mine could never get his back extended well in the deadlift start position. Eventually we noticed his knees were in when he was setting up. Once we cued “knees out” and he set up again his back position immediately looked perfect, it was like night and day. Very useful and under looked advice.
