You are clearly lifting your heel and shifting your weight on to your toes, especially in the hole. Your weight should be evenly distributed on the balls of your feet and heels. As your heel leaves the ground it also looks like your feet are rotating slightly inward as well. The king of cues, "shove knees out" should help with this.
Consciously lifting your big toes will help to keep you further back on your feet as well. I did this for about the first three weeks and it really helped. Don't recall having to revisit this practice since.
Still garbage? Torso too upright? Knees moving around too much? Think I know the answers already...
