To my (non professional ) eye it looks like 1,2,4,5 are on the line. 3 is good.
Also they otherwise look ok. The only thing i'd mention is that your knees (and thus hips) move back a little right out of the hole when ideally they shouldn't.
I've been working pretty hard to get my form straight, and I feel that this is the best form I've done in a set.
