Seems like I might have been letting the bar come forward a bit at the bottom; I'll work on keeping the chest up. Any other thoughts?

and was told to slow my descent. I think it is slower but perhaps still a bit too fast.

You actually need to work on keeping your chest down. I kid a little. However, your knees come too far forward and you are working too hard to stay upright. Lean over a little more. Widen your stance slightly. Point your toes out at 30 degrees. Drive up with your hips the whole way, even through the middle.
