Default Squat Form Check - Hip Flexor Pain

I started training back in August with and old rack someone gave me.
12/27/16 was the last day I was able to complete my sets. Squat 250x5x3 Since then my right hip flexor has been giving me pain particularly when I sit and lay down, I can bend and fee squat with nearly no noticeable pain. I tried to resume today and could only manage to 225x5x3, any higher weights and I felt like I would have collapsed under the bar.

Hopefully I followed the guidelines well enough to keep my life.

That looked easy. You are not quite hitting proper depth; can't tell why from this video (could be stance, could be you are just mis-judging). Lean over more right from the top/as soon as you start the descent. Set your knees in place over your toes by about 1/2 way down, then use your hips to complete the descent and start the ascent.

Nothing horribly egregious here that would indicate hip flexor problems.

Perhaps the cause was improper deadlift? If it helps any I think the pain is specific to the Psoas Major.
