Your stance is too wide. Bring your heels under your shoulders. Get leaned over more at the start and stop looking at yourself in the mirror. Find a spot on the ground 4-5 feet away.
Save up for a good belt.
For the power clean, hang out over the bar with your torso a little longer. You're in a rush to get behind the bar.
Be more explosive when the bar hits the right spot.

Coach, I tried your recommendations for the squat today for my working set, but it obviously didn’t go to well. The narrow stance felt much better, but looking down was very difficult. How should I approach getting used to this position? Also, when you say stay leaned over at the start, do you mean when I'm fully locked out or when I start descending?

Give yourself a small target to stare at.
When you start descending, you immediately want to lean over and point your boobs at the ground.

Do I lower the weight due to the form creep? If so by how much?

If you're talking about the squat, you're probably fine to re-run 315. It didn't look too difficult for you.
