I would appreciate a form check of my squat. I am mostly concerned about my back position since I have some minor pain on my left shoulder, from a possible shoulder Impingement that developed while I had shoulder and elbow tendonitis.

They are off the ground. Indicates that the bar is forward from my midfoot leading to, maybe extra effort to my shoulders to keep me grounded?
Does it look like I must push my bottoms back even more, adjust the bar position on my back or something else?
Your grip is CRAZY. It looks like it's almost resting on your wrist. Narrow your elbows and widen your hands: the bar should be IN your hands, you should be able to squeeze it. This will get your upper back more engaged as well: it's a bit soft.
