From looking I saw my head's too high, push out around the knees and sink my hips into the lift a bit.

With the camera set so low, it is difficult to see everything that is going on. Focus on "step 1" of the deadlift setup. Although the plates are blocking our view, you seem to be setting up with your shins too far from the bar. Walk up close to the bar so that your shins are only one inch away. When looking down while standing, the bar will visually be over the tongue of your shoe in most cases. Also, turn your toes out more. Make sure that you DO NOT MOVE THE BAR when you finish the setup.
You need to set up with the bar over your mid-foot, then put your hands on it without bending your knees. Then bend your knees until the shins touch the bar without moving it. Then lift your chest without dropping your hips. Then drag it up your legs. You're not really doing any of those things.
