Help with Squat form
I filmed myself squatting from behind for the first time last week and realized things did not look as they should. I noticed 3 problems
1. My grip on the bar becomes uneven during the lift, with my left elbow unable to get as far back as my right elbow, and my left wrist bent into extension while my right wrist comes over the bar
2. The bar is uneven and leans to the right side. Interestingly I cannot feel myself leaning at all when I squat, the bar feels balanced.
3. On my way back up my hips shift to the left and my knees both point to the right in what almost looks like a rotational movement out of the hole.
I did injure my back a couple of years ago, pulling a muscle in the right side of my lower back. This took several months to heal, and I still have residual pain some days from it. I am wondering if my body adapted to try and avoid using those muscles during the squat, hence the shift to the left?
I would really appreciate any help in correcting these form problems. I have attached the video below, it was my 3rd set of the day at 265.

Your descent is too far below parallel, and too fast--it's close to an uncontrolled descent (caution: I'm not a coach!).
You'll need to post video from a better angle to get more meaningful feedback from the coaches:
