The bar needs to be in contact with your shins when you pull. Then you need to drag the bar up the legs. Your torso angle looks okay. Just keep the bar closer. Also, gain weight.

Your hips are rising before you make the pull. If you have starting strength 2nd edition take a look at p131-133. It explains that this is also the reason why the bar is moving away from you.
The bar is in front of the mid foot.
You are straightening your legs without the quads contributing to the pull, which is what is making the bar be so far away from your body. Like Tom said, the bar has to drag up the legs the whole way. The best way to do that is to think about keeping your knees in place and push the floor away with your feet. Thats how you start the pull.
